Your child’s registration into our school community is an exciting time in your family’s life. We look forward to sharing this experience with you!
In an effort to facilitate the registration of students into the Beachwood City Schools, you will be using an on-line registration process. Most of the information and documents that the school district requires for registration will be submitted to the District Registrar through the link to this on-line registration (OLR) program in Infinite Campus. As you enter the registration information into the OLR, you will be prompted to complete all required fields. In addition, you will upload your required documents directly into OLR. If you are unable to upload the documents, please contact our District Registrar, Grace Perryman at 216.464.2600 x244. All required documents must be submitted prior to final registration and all forms need to be FULLY COMPLETED.
Please read through the directions and details outlined in this letter before beginning and do not hesitate to call our District Registrar, Grace Perryman, at 216-464-2600 x244 with questions.
When registering your child, you will need to produce the following documents:

A picture identification is required at the time of registration. Please upload or bring ONE of the following:

2. Proof of Residency
To enroll in Beachwood Schools, you and your child must be Beachwood residents. Our school district needs to verify proof of residency for school attendance. After reading this page, please fill out the registration information in OLR and upload or bring proof of residency. Acceptable proof of residency documentation must include:
ONE of the following:
- Mortgage or deed
- Lease/rental agreement*
- Purchase agreement
*If you are renting a single-family home, the Certificate of Rental Occupancy from the City of Beachwood needs to be obtained and submitted along with your updated rental/lease agreement.
In addition, TWO of the following:
- Utility bill (gas, water/sewer, electric or land line phone bill)
- Tax return
- Voter registration
At registration, you will be asked to sign a sworn affidavit confirming your residency. If you and your children are living in the abode of another party, that party must co-sign the sworn affidavit. These documents must be presented at registration and the co-signer must be present at this time, if applicable.
3. Proof of Age
To attend Kindergarten, a child needs to be five-years-old by September 30th. Your child’s ORIGINAL birth certificate must be presented at registration. If you do not have a birth certificate, please contact the Bureau of Vital Statistics at 601 Lakeside Avenue in Cleveland at 216-664-3400, and obtain the birth certificate prior to registration.
4. Proof of Custody
If you are a single parent and have custody of your child, it is necessary to provide the court document that confirms proof of custody. These documents must be uploaded to the OLR or presented at registration.
5. Health History Forms and Medication Forms
The School Entrance Medical Physical Examination Form and the Immunization Report Form MUST be completed by your child’s pediatrician. They necessitate obtaining a current physical examination and updating of required immunizations for your child. These forms are due by the first day of school. Under Ohio school regulations, we cannot accept your child unless this information is provided with the exception of newly transferred families to the United States, in which case you will be given a 14-day extension.
The Medication Administration Forms can be found on the top-right pane of this page. These prescription/non-prescription medication authorization forms are to be completed only if your child is to be administered medication during the school day. These forms do not need to be returned until the start of school with the appropriate medication.
6. Consent for Records Release Form
To help in planning for your child, the school may contact your child’s preschool program to obtain a progress report. Please complete the information form that is on the right side of this page.
7. Special Needs Students
If your child is on an Individual Education Plan (IEP) or is a student with special needs, please provide the school with these records and contact the principal and/or the Director of Pupil Services at 216-464-2600 x235.
Once the online registration process is completed and AFTER you have provided the District Registrar with all required forms and documents, your child will be enrolled into the appropriate grade level and school building. Principals and counselors will be notified when you have completed the registration process. Please be sure to ask the District Registrar, Mrs. Grace Perryman, for additional information.
