Step 1
OPEN THE ONLINE REGISTRATION FORM
Please follow the prompts to fill out all mandatory fields in the online registration system and upload the necessary documents directly into the system. After completing the online application, please arrange to meet with the District Registrar to finalize your child's registration. A link to schedule this appointment will be provided within the online application.

Step 2
UPLOAD THE REQUIRED DOCUMENTS
1. Verification of Parent/Guardian
At the time of registration, you must present a valid photo ID. Please upload or bring ONE of the following:
- Current Driver’s License
- State Issued Identification Card
- Passport
- Military Identification Card
2. Proof of Residency
To enroll, proof of Beachwood residency is required for you and your child. Acceptable documents include:
- Mortgage, deed, or purchase agreement
- Signed lease/rental agreement
- Plus TWO of the following:
- Utility bill
- Home internet bill
- Tax return
- Voter registration
3. Proof of Age
For kindergarten, a child must be five years old by September 30. Please present the original birth certificate. If you do not have a birth certificate, please contact the Bureau of Vital Statistics at 601 Lakeside Avenue in Cleveland at (216) 664-3400 and obtain the birth certificate prior to registration.
4. Proof of Custody
Single parents must provide court documentation confirming custody.
5. Health Forms
The School Entrance Medical Physical Examination Form and the Immunization Report Form must be completed by your child’s pediatrician. These forms are due by your child's first day of school. Newly transferred families to the United States will be granted a 14-day extension in accordance with Ohio school regulations.
The Medication Administration Authorization Form should be completed only if your child is to be administered medication during the school day. These forms do not need to be returned until the start of school with the appropriate medication.
6. Students with Special Needs
For children on an Individualized Education Plan or with special needs, please provide these records and contact the principal or Director of Pupil Services at (216) 464-2600, extension 2217.
