Step 1: Registration Forms

Download and complete the appropriate Registration Forms posted to the column to the right.

Step 2: Legal Documents

At the time of registration, you will also need to provide us with:

Parent/Guardian Identification

One of the following:

Child/Children's
Paperwork

One of the following:

Current driver's license
State-issued identification card
Passport
Military identification card

Child's birth certificate
Custody papers (if applicable)

Step 3: Proof of Residency

The Beachwood Board of Education requires that proof of residency be obtained from all families registering their child(ren). Please bring the following information with you to your appointment:

One of the following:

Mortgage deed
Purchase agreement
Rental/lease agreement*

AND

Two of the following:

Tax return/bill/statement
Voter registration
Utility bills (water/sewer,electric, gas, phone-land line only)

Step 4: Health Forms

At the time of registration you will also need to submit any health-related forms (such as administration of medication) for your child. Please visit the Health Services page for additional information and to download the forms.

Step 5: Contact the Registrar

When all of your paperwork is completed, please email district registrar Vickie Diamond, or call 216/831-2080 x212 to schedule an appointment to enroll your child(ren).
